Online Forms

The Clearwater Public Library System makes two forms available online:

• Library Card Application
  Apply for a library card online.
• Request for Purchase Form
  We don't have a book in our catalog? Ask us to purchase it.
  
  
• The web-based Interlibrary Loan form has been discontinued. 
  Please use the "Request for Purchase" form for Interlibrary requests. We apologize for the inconvenience.
